Read these requirements carefully. Applications that do not meet instructions or that are missing required information may be delayed or rejected.
General Instructions
- Use typewriter or ink to complete the form.
- Sections I, II and IV are to be completed by the property owner.
- Section IV is to be signed by the property owner.
- Section III is to be completed and signed by the contractor who will be performing the work.
- Residential applications may require up to three business days for approval.
- Commercial, business, industrial, change-of-use, or other types of permits may require two weeks for approval.
- Fees for residential applications must be made in full at the time the application is submitted.
- Application fee for non-residential permits must accompany the application before calculation of I & I removal and connection fees.
- Application fees are non-refundable.
- Copies of inspection reports that fail will be forwarded to the owner and contractor. Passing inspection reports will be sent only if requested.

Owner Information
- All lines must be completed.
- Owner's address is the mailing address of the owner (not necessarily the property address).
Property Information
- Location of property: enter the street address for the property the application is being made for.
- Map and lot information may be obtained from the City of Biddeford Tax Assessor, Building Inspection or Engineering Departments.
- Check item “a” for replacement of a damaged sewer lateral or service. If a new connection is made to the sewer main, the applicant must pay a $400.00 sewer connection fee.
- Residential sewer fees are based upon the number of bedrooms (up to three bedrooms = 200 gpd; additional bedrooms = +90 gpd each).
- For I & I removal fee calculations, each apartment/unit in a multi-family home is treated as a single-family home. Multi-family permits pay one application fee, one connection fee, and one inspection fee (separate buildings may incur separate fees).
- Business, commercial, industrial, change-of-use, and other permit types may require additional information (e.g., business type, employee count, water use records, parking spaces, seats, meals served).
- Industrial permits require an Industrial Pretreatment Program (IPP) Permit. Obtain this from the IPP Specialist at the City of Biddeford Wastewater Treatment Facility (Water Street). IPP Specialist: (207) 282-1350.

Important
Do not begin work until the proper sewer permit has been issued. Incomplete applications or missing information will delay processing.
