General Instructions

Each license will require the applicant to complete required information fields and upload copies of any required documents. Please ensure you have all the required information and documents ready to upload prior to beginning the online application process. Applications submitted without the required documentation may delay the review process or cause the application to be rejected.

Application Instructions

To select a license application, use the search box below. License use will list various licenses under each type. You may also scroll through the list. After ensuring you have all the required information for upload, click the “Apply Online” link for the required license.

*All Business and Liquor Licenses expire June 30th of each year. All new applications will be prorated the first year.

-BUS LIC CLASS A, to be selected for businesses inside the city limits, $25 per year.
-BUS LIC CLASS B-OUT OF COUNTY, to be selected for businesses located outside of Polk County and are traveling into the city limits of Bolivar to conduct business, $75 per year.
-BUS LIC CLASS B-POLK CO, to be selected for businesses located inside Polk County; but outside of the city limits of Bolivar, and are traveling into the city limits of Bolivar to conduct business, $50 per year.
-DAILY BUSINESS LICENSE, $10 per day.
-WEEKLY BUSINESS LICENSE, $20 per week.
-FIREWORKS LICENSE, $25.

Required Information

*Businesses that collect sales tax: Must provide a copy of a non-expired State of Missouri Sales Tax – No Tax Due Certificate. If you have not established a Missouri Sales Tax Number or need the No Tax Due Certificate, please reach out to Missouri Department of Revenue via the department’s website or by phone at 573-751-4876. Also Note: MDOR will not be issuing Sales Tax Rate Change Notifications. Each rate change will be noted on the MDOR website at https:dor.mo.gov/business/sales at least 15 days before the effective date.
-Sales Tax ID Number: Needs to be entered on the application under Step 3, EIN/Tax Number. If applicant does not collect sales tax, you need to enter N/A in the field.

*A copy of Workers’ Comp Insurance must be provided by all businesses with five or more employees, pursuant to RSMo. 287.030, and by contractors or sub-contractors with one or more employees, or businesses should submit a Notarized Affidavit of Exemption for Workers’ Compensation Insurance Pursuant to RSMo. 287.061.

*The business acknowledges that Section 285.530 RSMo. Prohibits any business entity or employer from knowingly employing, hiring for employment, or continuing to employ an unauthorized alien to perform work within the State of Missouri. The Business also affirms that, to the extent required by State and Federal Law, it is in compliance with requirements to verify the legal status of its employees through E-Verify or similar such services under the Immigration Reform and Control Act of 1986.
